All athletic paperwork is now submitted online (link above)! Below are links to the our Athletics Policies and the physical you need to take to your doctor, but all paperwork is now submitted online
DIRECTIONS FOR COMPLETING THE ONLINE CLEARANCE
PHYSICAL FORM
Please print all 3 pages and take them with you to your physical. All 3 pages need to be scanned and uploaded for the online clearance. You can also pick up a copy from the main office.
ATHLETIC ELIGIBILITY POLICIES
You do NOT need to print this and turn it in, this is embedded in the online clearance. Includes CIF, GUSD, and GHS rules and expectations.
STUDENT-ATHLETE DRUG TESTING POLICY
You do NOT need to print this and turn it in, this is embedded in the online clearance.
ADDITIONAL FORMS:
Student Transportation Release Form
Form for a student athlete to return home from a contest with a non-parent/guardian adult.
Volunteer Driver Registration Form
Form for parents or other adult volunteers to help transport kids to and/or from athletic contests.
Student Accident & Health Insurance
This is a low cost medical/dental accident insurance program for students injured on school premises or during school activities.
